Location: Bronx NY 10461
Contract: 13 Weeks
Hours: 40 per week Day Shift 10 AM 6 PM
Department: Emergency Services Care Management

Weekly Gross: $2779.87

Overview
This position supports Emergency Department operations through care coordination discharge planning and utilization management to improve patient throughput and transitions of care.

Responsibilities

Manage care coordination for ED patients
Conduct utilization reviews and discharge assessments
Partner with clinical teams to reduce length of stay
Arrange post-discharge services and resources
Ensure documentation meets regulatory guidelines

Qualifications

NY RN License (active)
5 years clinical RN experience
Experience in discharge planning and utilization review
ASN/ADN required
BLS required

Preferred

Bachelors or Masters degree
Case Management background
