Job Summary
Green Elms Surgery in Jaywick are currently seeking an experienced practice nurse to work on a part-time basis for 10 hours per week.
Working as a member of the practice multidisciplinary team the post holder will ensure nursing services are delivered effectively to the entitled patient population.
Main responsibilities will include;
- Management of clinical areas such asinfection prevention and controlhealth promotion chronic disease management health promotion well women and well man clinics.
- Support the practice management team in the reviewing and delivery of clinical policy and procedure.
- Support the production of the IPC annual statement in conjunction with the nurse manager
- Develop implement and embed an effective care management programme for the frail and housebound patients registered at the practice
- Identify manage and support patients at risk of developing long-term conditions preventing adverse effects to patients health
- Implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing
- Diagnose and manage acute and chronic condition
- Co-ordinate the provision of travel medicine services
- Request pathology tests and process results

About the Candidate
All applicants should have the following qualifications registrations skills and experience:
Essential:
- Registered Nurse
- Experience of working within a primary care
- Experience of chronic disease management
- Relevant clinical skills listed above
Desirable:
- Experience working as a practice or community nurse
- Mentor or teaching qualification
- Family planning qualification
