POSITION INFORMATION
- Position summary:
  - The Hospice Social Worker is responsible for providing initial psychosocial assessments plan of care ongoing psychosocial counseling bereavement services community education and outreach and referral to Hospice patients and families.
MINIMUM QUALIFICATIONS
- Education:
  - Masters Degree in Social Work and is a Licensed Independent Social Worker.
- Required length and type of experience:
  - Minimum of three (3) years of clinical work experience preferably in a healthcare setting or other related field.
- Required licensure certification or registry:
  - Licensed Independent Social Worker.
