Description3034037: Licensed Wellness Self Management Specialist -Psych Admin - Rivington - Full Time Day
Job Summary:
To be cognizant of the philosophy standards objectives and policies of the department and the hospital.
ResponsibilitiesDuties and Responsibilies:
Clinical/Technical/Service
- Works with program psychiatrist in assessing medical/psychiatric needs of program
- patients.
- Provides linkage and liaison with primary care provider.
- Provides health evaluation and follow-up to referred patients management and
- administration of medication under supervision of MD.
- Evaluates routine urgent health problems and facilitate coordinate referrals evaluate
- laboratory reports.
- Provides health mental education treatment rehabilitation and support services to
- referred patients. Services may be provided in the community as needed.
- Collaborates with program nurse and team members to develop and implement
- treatment plans involving multiple modalities.
- Participates in team meetings/rounds.
- Documents medical record(s) in a timely and accurate manner.
Organizational/Managerial
- Participates in the departments performance improvement activities.
- Maintains patient/employee confidentiality in the management of information.
- Observes the Health Care Systems Compliance policies.
Educational/Professional Development
- Participates in the development of other staff members.
- Meets regulatory licensure and annual health assessment requirements.
- Identifies learning strengths and needs.
- Utilizes learning resources.
Communication/Relationships
- Demonstrates a professional courteous and respectful attitude in dealing with
- patients families and significant others.
- Displays courtesy tact and patience during interactions with all members of the hospital
- staff and extended community.
QualificationsRequirements:
- Bachelors degree in Nursing
- Prior nursing experience with the mentally ill with co-morbidities of substance abuse.
- Licensed Registered Nurse in NYS preferred; LPN considered.
- Certification in Psychiatric Nursing and/or Chemical Dependency preferred.
