H.S. Diploma or Equivalent required
Three years relevant experience preferably in a healthcare or service environment required
Patient Care/ Customer Service / Healthcare experience strongly preferred
MS Outlook MS Office (Word Excel) internet navigation clinically based software and financially based software
What You Will Do
Responsible for coordinating the clerical and administrative processes for entity care transitions management
Responsible for managing departmental mailbox phone lines and/or fax
Supports transition planning activities as needed under the direction of care transition managers
Calls vendors physician offices clinics etc.
Faxes (electronic and traditional) documents as needed
Makes copies or gathers other data as needed
Collect and enter data as needed
Completes other tasks and projects assigned
