Description:

Responsible for scheduling clients Home Health/Hospice services with clinician continuity. Identifies client need and schedule clinicians with the appropriate competencies to match client level of care. The Scheduler is expected to exercise judgement in determining the most effective and efficient method of scheduling and rescheduling patients. Provide optimal levels of communication with patients clinicians clinical managers physicians and homecare coordinators. May also provide coverage for multiple sites and/or Hospice support.

May also provide coverage for multiple sites and/or Hospice support working remotely or hybrid. Must be local to Philadelphia.

Requirements & Skills:

  • High School diploma or GED
  • Must have a valid drivers license
  • Home care or hospice experience a plus
  • Experience in Epic (EMR) a plus
  • Strong critical thinking and problem-solving abilities
  • Competent computer skills and ability to navigate electronic systems
  • Excellent organizational and time-management skills
  • Strong interpersonal and communication skills
  • Ability to foster and maintain positive relationships with referral sources
