Hours: 40 hours/week
Position Overview
A healthcareis seeking a Licensed Social Worker (LMSW) for a short-term contract assignment. The Social Worker will provide psychosocial assessment care coordination and discharge planning support to patients while collaborating closely with interdisciplinary healthcare teams.
Responsibilities
Conduct psychosocial assessments and develop patient care plans
Provide counseling crisis intervention and emotional support to patients and families
Coordinate discharge planning and connect patients with community resources and social services
Advocate for patients psychosocial needs within the healthcare system
Collaborate with physicians nurses case managers and ancillary staff
Maintain accurate and timely documentation in the Electronic Medical Record (EMR)
Manage a high-volume caseload in a fast-paced clinical environment
Ensure compliance with hospital policies regulatory standards and ethical guidelines
Compensation
Gross Weekly Pay: $1900/week
W2 Rate: $18/hour
Weekly Stipend: $1225/week
Required Qualifications
Masters degree in Social Work (MSW) from an accredited institution
Active New York State LMSW license (required)
Minimum 6 months to 1 year of experience in a medical or healthcare setting
Strong knowledge of community resources and social service programs
Excellent communication assessment and interpersonal skills
Ability to manage a large caseload effectively
Epic EMR experience preferred
Proficiency with Microsoft Office Suite
