Job Summary

Our client is seeking an experienced Primary Care Physician for a value-based care opportunity focused on delivering high-quality patient-centered care to senior populations.

This physician-led mission-driven organization is nationally recognized for transforming care delivery for patients with multiple chronic conditions. The model emphasizes quality over volume strong provider support and exceptional work-life balance.

Role Highlights:

100% outpatient primary care

Value-based care model

Average of 18 patients per day

Small patient panel capped at approximately 450 patients

Dedicated care team including Medical Assistant and Scheduler

Administrative time built into the schedule

No nights weekends or call

MondayFriday schedule

Clinic hours: 7:30 AM 5:00 PM (last patient scheduled at 4:15 PM)

Compensation & Partnership:

Base salary: $250000

Bonus: Up to $20000 in Year 1 increasing upon partnership

Partnership track with no buy-in

Typical partnership timeline: 1218 months

Approximately 65% achieve partnership within one year

Partner compensation exceeds $300000 annually

Patient Population:

Seniors with an average of 35 chronic conditions

Long-term relationship-based care model

Additional Practice Support:

In-house consulting specialists (including Cardiology and Podiatry)

Holistic services such as Acupuncture Tai Chi Yoga and Nutrition

Door-to-door transportation provided for patients at no cost

Requirements

Residency trained in Internal Medicine or Family Medicine REQUIRED

Board Certified in Internal Medicine Family Medicine or Geriatric Medicine REQUIRED

Active and unrestricted medical license required (any U.S. state)

Relocation candidates accepted

Commitment to value-based care and senior-focused medicine

Ability to manage a chronic care population in an outpatient setting

Spanish language skills preferred but not required
