Tasks
- Provide compassionate person-centered care to adults with a variety of needs ensuring their comfort and safety.
- Assist service users with daily activities such as cooking cleaning bathing and dressing while promoting independence.
- Communicate effectively with service users their families and other healthcare professionals to ensure coordinated care.
- Maintain accurate records of care provided including observations and any changes in the service users condition.
- Participate in ongoing training and development programs to enhance skills and stay updated on best practices in home health care.
Requirements
- Previous experience in home health care or related field is preferred.
- Ability to provide compassionate person-centered care with a focus on the individual needs of service users.
- Excellent communication skills and the ability to work effectively within a team.
- Flexibility to adapt to varying schedules and tasks as needed.
- Commitment to maintaining high standards of honesty fairness and equity in all interactions.
