Our House ofPortlanda program apartof CascadeAIDSProjectis excited to announce that we are hiring skilled and team-orientedNeighborhood Housing Care Program (NHCP) Housing Case Worker.The NHACP Housing Case Worker role is to support our clients obtaining andmaintainingstable housing. NHCP clients may experience a wide range of challenges to their wellbeing including HIV/AIDS a history of substance use and mental health Housing Case Worker will serve as a liaison between clients and property management and funding as an advocate and motivator whileassistingclients with building the fundamental skills of responsible renting.
This is aFull-Timerole working at Our House of Portland inSE schedule for this role isMonday-Thursday 8:30am-5:00pm. This is a hybrid role3 days in office and 1 day work from home.
Thepayfor this position is$34.51which is equivalent to just over$57k ayear. TheNHCP Housing Case Workers pay increases withtenureand the top of the pay scale is$ is a union represented position so the compensation benefits and conditions of work are collectively bargained.
Who You Are
GED or High school equivalent
Must be able to pass a DHS background check. (Potentially Disqualifying Conviction List)
Current unrestricted OregonDriversLicense
Ability to be insured under employers insurance.
Successful experience working with ethnic racial economic and sexually diverse populations and persons who have experienced houselessness persons with a mental illness and/or substance use issues.
Experience working with people living with HIV/AIDS (PLWHA) and/or people with disabilities chronicillnessand/or other barriers to employment. A strong understanding of issues relevant to people living with HIV communities of color and the LBGTQ community and how those issues affect or influence service delivery.

Assistindividuals inidentifyingbarriers to obtaining/maintainingstable housing
Assistindividuals in developing their own plan for defining and advancing in their path to wellness with person centered care harm reductiontechniquesand therapeutic boundaries.
Works in collaboration with NHCP clinicians clients primary care team and community partners to providewraparound services.
Transport clients using a company vehicle to medical appointments housing-related meetings grocery shopping and other goal-related activities
Develop andmaintainknowledge of community housing resources through training and independent research
Identifyopen housing waitlists and opportunities;assistclients with application processes when appropriate
