Job Summary

Job Title: School Adjustment CounselorReports to: Executive DirectorStatus:ExemptSalary Range: $65000-$75000Primary Responsibility: The School Adjustment Counselor provides social-emotional support and counseling services to students helping them overcome personal social and behavioral challenges that interfere with academic success. This role collaborates with teachers administrators families and community resources to promote student well-being and a positive school Duties:In all areas perform other related duties as required and/or collaboratively with the Education Director Clinical Director and Dean of Students to support the academic behavioral and social-emotional needs of students. Develop and implement intervention plans to support students mental health and academic a clinical caseload of day students with potential involvement with residential education individual and group clinical support within the school setting using a trauma-informed care and consult with teachers and instructional assistants regarding students clinical needs behavior regulation and trauma-responsive with families of day students including conducting home visits as clinically with outside providers and agencies (therapists DCF outpatient clinicians placements) to ensure continuity of implement and monitor individualized behavior support plansContribute to the development of IEP goals and objectives and complete quarterly progress reportsAttend and actively participate in IEP meetings case conferences and multidisciplinary team meetingsCollaborate with medical providers and attend medication reviews as neededSupport transition planning for students entering or exiting the programSupport students during school activities including field trips and community-based programmingParticipate in Extended School Year (ESY) and summer programmingConduct risk assessments and crisis intervention when accurate and confidential records in compliance with school policies and state in IEP meetings and contribute to the development of social-emotional goals for students with special staff with de-escalation strategies and post-crisis and analyze behavioral and clinical data to inform interventions and measure student in program development and continuous quality improvement initiativesEducation: Masters degree in Social Work Counseling Psychology or related field (in process acceptable)Qualifications:State licensure (pending acceptable) or certification as a School Adjustment Counselor or working with children and adolescents in an educational communication problem-solving and crisis management of trauma-informed practices and cultural :Maintain active certification in First Aid CPR CPI Non-violent Crisis Intervention and attend all other required trainingsMust have a valid drivers licenseSkills/Abilities:Communicates effectively for administration and management case management services community and inter-agency relationsDemonstrates leadership skills maturity and ability to deal effectively with emotionally demanding and stressful Requirements:Must demonstrate good judgment in decision making and determination of information that should be communicated to staff and other supervisorsMust fully accept demonstrate and represent Nazareths stated mission vision and values in daily workAbility to read write and speak English; Bilingual capacity in English/Spanish an assetMust be able to communicate effectively by telephone and in person. Must be able to record clear messages and keep accurate be proficient in Word Excel PowerPoint and can quickly learn any needed new computer skills Prepares thorough accurate and understandable communications and reports in a timely mannerAbility to provide direct coaching and supervision of employeesMust be able to communicate effectively by telephone or in have manual dexterity and motor skills to grasp and lift objects of up to 25 be able to bend assist in lifting a person twist and sit on floor for implementation of daily be able to physically escort and physically restrain be able to bend and lift up 25 lbs. to purchase and/or access program be able to climb for entry and mobility throughout the buildings which may have several levels of demonstrate maturity and ability to deal effectively with emotionally demanding and stressful be able to maintain and exercise confidentiality at all be able to travel to various sites to attend meetings conferences and to achieve a satisfactory competency and performance evaluation annualProcedure: Date:Approved by Board of Directors: 12/2025Distributed to Staff: 12/2025
