Schedule: 40 hours per week Monday Friday 8:00am 4:30pm
What Youll Do in This Role:
The Community Care LPN at Huron Valley PACE works closely with the interdisciplinary team to support frail elders with complex needs. This role uses critical thinking and a proactive care approach to enhance participant independence ensure coordinated services and promote the highest quality of life.
Primary Responsibilities:
- Provides coordinated medical care including wound care medication administration participant education and service support.
- Works with participants families and the Interdisciplinary Team to develop and follow care plans.
- Encourages participant independence to maximize functional capacity.
- Monitors changes in participant condition and communicates updates to the team.
- Utilizes critical thinking to assess needs and implement safe comprehensive care.
- Maintains accurate and timely documentation in accordance with PACE policies.
- Advocates for participants and assists in resolving concerns or barriers to care.
- Requires flexibility in scheduling to support participant and caregiver needs.
- Works in participant homes with exposure to various environmental conditions.
- Drives within the service area and must have reliable daily transportation.
What Youll Need:
- Licensed Practical Nurse (LPN) with a current Michigan license.
- Current drivers license reliable transportation and acceptable driving record.
- BLS or CPR certification (must be obtained within 3 months of hire if not current).
- Ability to lift and move objects up to 35 pounds as needed.
