Job Duties
The Population Health Manager provides or coordinates health improvement planning services in the local health district by planning implementing and evaluating health education activities and working closely with others to fulfill health improvement needs and goals. Typical duties may include: preparing and submitting grant applications related to health improvement projects delivering educational programs to clients and staff conducting health education trainings revising and developing educational materials collecting program-specific data and writing reports conducting needs assessments and facilitating the CHIP process providing technical assistance to local and community agencies/organizations regarding program planning serving as public contact or PIO about services and activities and preparing news releases and coordinating district media contacts and activities. May include the supervision of staff. Serves as a member of the districts Management Team working closely with Health Director and Team to achieve VDH mission.
Minimum Qualifications
Considerable knowledge and experience in the principles and practices of health education community health assessment and improvement planning and community organization and engagement
Experience performing research and utilizing quantitative and qualitative methods for assessing data and programs
Experience with project management development of goals and objectives work planning and coordination of activities and initiatives
Experience with grant management grant writing and grant activity reporting
Experience preparing and delivering media updates informational materials and presentations to a variety of audiences
Excellent communication skills verbally and in writing to effectively manage interpersonal and interorganizational communication and coordination networks
Proficiency in the use of standard Microsoft Office products
A. Performance Management (for employees who supervise others)
Serves in a managerial/supervisory capacity and conducts performance management activities for staff
Establishes and reviews work assignments and priorities and resolves related issues in conjunction with program management and Human Resource staff
Ensures relevant training and workplace safety for staff and conducts appropriate performance monitoring and appraisals
B. Project Management
Provides strategic data driven management of district population health efforts
Directs planning development implementation and evaluation of processes for community health assessment and improvement planning community organization engagement and participation and other identified needs for capacity building
Assesses and monitors status of partner organizations engagement with community health assessment and improvement planning processes
Prepares project and training work plans for health district and related agencies
Schedules and arranges opportunities events and meetings
Prepares annual CHA-CHIP training in accordance with needs priorities and deadlines
Serves as liaison for health district and their partner agencies on matters of CHA-CHIP
Coordinates and supervises assessment and improvement activities and ensures that team members deliver expected outputs
Ensures compliance with required elements of funding source provides leadership in the writing editing and submission of reports on CHA-CHIP goals and outcomes
C. Resource Management & Documentation Review
May monitor budget and expenses to ensure resources are appropriately used
Procures supplies such as incentives texts materials etc.
Assures health department records are secure and follows HIPPA regulations at all times
Prepares grant applications; coordinates grant activities and prepares grant activity reports
Applies to national and local sources for project funding; actively seeks funding for support enhancement and additions to projects and training needs
Verifies data for accuracy and identifies trends develops projections prepares reports as directed and follows established procedures
Provides high level oversight of media interactions including social media and communications with media partners
D. Leadership & Community Outreach
Serves in a lead role to identify needs to conduct effective community health assessment and improvement planning processes; establishes strong working relationships in communities and assists with development of community relationships
Clearly communicates purpose goals and objectives to peers and beneficiaries to clarify roles and responsibilities for completing community health assessment and improvement planning processes
Encourages creative problem-solving among partners to enhance productivity and effectiveness
Promotes integration consistency and collaboration toward collective impact among services across VDH services within the health district
Builds effective working relationships with all disciplines within VDH and appropriate outside agencies
Additional Considerations
Previous experience in public health and the assessment/improvement process preferred
Certified Health Education Specialist (CHES) for Health Educators preferred
