As a Physician Assistant or Nurse Practitioner youll join a purpose-driven team that:

Continuum-Based Care (Clinical Excellence):Deliver high-quality care across home senior living and community settings supported by a multidisciplinary team in palliative psych and neurology.

Your Responsibilities Include:

Delivering personalized high-quality care to seniors in their homes assisted living hospice and nursing facilities.
Delivering dementia-informed relationship-based care across the continuum of care.
Conducting comprehensive assessments and developing tailored care plans for older adults.
Managing chronic conditions and performing care with skill and empathy for patients and their families.
Collaborating with cross-functional partners including palliative psych and neuro to deliver coordinated whole-person care.
Building trusted relationships with residents caregivers and facility partners to enhance engagement and referrals.
Maintaining consistency in documentation follow-through and patient communication.

What Youll Bring

Active unencumbered NP or PA license in the state you will be practicing.
Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred).

Valid drivers license and reliable transportation.
