Summary The AFC Care Manager provides psychosocial and counseling assistance to caregivers and individuals enrolled in the Incompass AFC Program Essential Job Functions - Responsible for the completion of the initial and ongoing assessments of the AFC qualified setting
- Obtain a social history and conduct a psychosocial assessment and evaluation
- In conjunction with the Registered Nurse responsible for the selection training evaluating and supervising of the AFC caregivers
- Participate in the development implementation and ongoing review of the AFC plan of care
- Conduct on-site visits with each member at the qualified setting bimonthly (alternating with the bimonthly visit by the registered nurse for Level 1 and monthly for Level II
- Complete a care-management progress note corresponding with each on-site visit or encounter and upon significant change
- Review the AFC caregiver logs
- Assist with obtaining information and accessing other healthcare and community services
- Review and document the fire and safety procedures for the qualified setting
- Participate in discharge planning and implementation and reviewing at least annually the suitability of the qualified setting
- Conduct oneself in a professional manner consistent with the agencys values program mission and service delivery goals.
- Monitor the safety and well being of the Individual served and his/her family. Reports any issues to supervisor.
- Familiarize oneself with and abide by agency policies and procedures
- Serve as a Mandated Reporter.

- Social workers License from the Massachusetts Board of Registration preferred and 2 years recent experience working with elderly or adults with disabilities or bachelors degree and 2 years clinical experience in the care of elderly or persons with disabilities
Language Skills Ability to read and interpret documents such as letters from social security operating and maintenance instructions and procedure manuals. Ability to write routine reports and correspondence and speak effectively before groups is required. Must be able to write case notes in a clear and concise manner. Must be able to speak effectively to persons with developmental disabilities and communicate with families in a caring and clear manner. Bi-Lingual encouraged to apply! Physical Demands The physical demands described here are representative of those that must be met by an incumbent to successfully perform the essential functions of this job.
