Job Summary:
The Registered Nurse will provide skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Responsibilities include patient assessments medication administration treatment planning and collaborating with the interdisciplinary team. Strong communication and collaboration with patients providers and team members are critical.

Required Skills & Certifications:
- Clinical Assessment Medication Administration and Treatment Planning skills
- Minimum two years of hospital home health or hospice care experience
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Active Registered Nurse license in the state of working
- Bachelors degree in Nursing or related field
- Drivers license car insurance and automobile for field work
Preferred Skills & Certifications:

- Weekend availability is required
- On-call is required
- RTO restrictions must be included at the time of submission
Scheduling:
- One weekday shift coverage per week from 1600-0700
- One three-day weekend (Fri-Sun) a month (Fri 1600-0700 and Sat Sun 0700-0700)
- Start date: ASAP
