Your Role at a Glance

Youll serve as the primary nurse for an assigned caseload delivering high-quality care across the home health continuum by:

• Completing start-of-care routine and follow-up nursing visits

• Performing comprehensive patient assessments

• Developing and updating individualized plans of care

• Monitoring patient progress and identifying changes in condition

• Communicating with physicians clinical leaders and care teams

• Documenting care accurately and timely in the EMR

• Ensuring compliance with state federal and agency guidelines

• Maintaining patient privacy and HIPAA compliance

What Were Looking For

• Active unrestricted RN license

• At least 1 year of nursing experience in a clinical setting

• Associates or Bachelors Degree in Nursing preferred

• Home Health or Hospice experience preferred

• Experience with HomeCare HomeBase a plus

• Reliable transportation valid drivers license and auto insurance

• Ability to travel within the service area (approximately 50%)
