Job Summary

Position Summary:

Implements the agency program of full time professional nursing service to patients in their homes under the supervision of the Clinical Services Supervisor in accordance with agency standards and policies.

Essential Functions and Responsibilities:

1. Initiates and provides medically prescribed skilled nursing care and/or health counseling that contributes to the treatment of the medical condition achievement of the rehabilitation potential and/or reduction of pain and other symptoms.

2. Makes the initial visit to assess the patient s physical and emotional status needs and resources through observation interview and analysis of records.

3. Prepares the nursing plan of care in accordance with the physician s plan of treatment based on problems identified in the nursing assessment and provides patient teaching using the Care Plan/Standard of Care.

Requirement description : Coverage area: This will be covering the West side. Petoskey East Jordan Boyne City Harbor Springs Mancelona areas. Potential for Mackinaw City to Mancelona i-75 west.

Shift notes: The position would be a hybrid of scheduled visits and on call; hence the 15 hours. We would schedule some visits at the beginning of the shift the rest would be as needed or death visits. Generally we have staff doing visits for the first few hours and the rest of the time is call. So we could say 8 hours work with the remaining time being oncall. We re going to give them visits to do in the evenings up to a point but most will be oncall.

CERTIFICATION REQUIREMENTS :

BLS (AHA)

STATE LICENSE REQUIREMENTS :

Michigan

ADDITIONAL LICENSE REQUIREMENTS : Active at time of Submission
Weekend Requirements : With it being 7 on 7 off on call position it is every other weekend and all on call.
