Schedule: Monday through Friday with on-call weekends
Pay: Base pay of $40 per hour
Position Summary: Trinity Grove is seeking a dedicated and dynamic Neighborhood Coordinator to join our team. The primary purpose of this role is to supervise the day-to-day activities within the assigned neighborhood ensuring that the highest degree of quality care is maintained at all times. The Neighborhood Coordinator will work closely with various departments to establish a framework for person-centered resident care in alignment with the philosophies and objectives of Trinity Grove.
Key Responsibilities:
- Assist in directing the day-to-day functions of neighborhood activities including planning services programs and activities.
- Participate in the facilitys Quality Assurance and Performance Improvement program attending committee meetings and preparing reports as requested.
- Perform administrative duties such as completing forms reports evaluations and investigations.
- Provide leadership to personnel including coaching training and mentoring new employees developing consistent work assignments and completing performance evaluations.
- Conduct daily rounds observations and assessments.
- Demonstrate knowledge of emergency care disaster and evacuation policies and procedures and safety programs.
- Review resident records periodically to ensure completion accuracy and adequate reflection of the residents care and condition.
- Consult with physicians in providing resident care and participate in resident care plans.
- Meet with residents and/or family members as necessary following up on grievances and ensuring all residents are treated with dignity kindness and respect.
- Participate in the infection control program and ensure established safety guidelines are followed.
- Honor resident choice allowing residents to direct their own care and make personal choices regarding daily schedules and activities.
- Reflect Trinity Groves mission vision values and customer service standards in daily interactions.
Qualifications:
- Registered Nursing degree from an accredited college or university or a graduate of an approved RN program.
- Licensed registered nurse in the state of North Carolina.
- CPR certified or ability to become CPR certified.
- One to two years of supervisory experience preferred.
- Strong communication skills and ability to maintain confidentiality.
- Computer skills preferred; willingness to learn electronic documentation.
Working Conditions:
- Physical exertion involving pushing pulling and lifting up to 35 pounds; occasionally lifting weights up to 50 pounds.
- Prolonged standing and/or walking during which time objects are transported.
- Occasional bending squatting or reaching repeatedly.
- Ability to assist in the repositioning and transferring of individuals of varying weights following established safe lifting procedures.
