Davis Behavioral Health (DBH) is actively recruiting a full-time Case Manager to work on the Adult Mental Health Outpatient team. This position will be based out of the DBH Main Street Clinic office; however the majority of the work will be conducted in the community throughout Davis County.

As part of a multi-disciplinary team the Adult Mental Health Outpatient Case Manager is a bachelors level mental health professional who provides targeted case management behavior management skills development and personal services to adults with serious mental illness and co-occurring substance use. The case manager assists clients in effectively utilizing available community resources. Case management can be provided in the home office and community settings. This position offers hope guidance and skills toward obtaining and maintaining thriving independence in community living.

Essential Job Functions

The Adult Mental Health Outpatient Case Manager must be able to complete the following essential job functions:

  • Complete DLAs (or other agency identified case management needs assessments) at the required frequency in order to create individualized goals and objectives.
  • Use information from the DLA to complete individualized service plans in support of overall mental health treatment goals.
  • Maintain agency standards for productivity and documentation.
  • Engage with clients treatment teams to support clients and coordinate treatment objectives.
  • Maintain an active caseload.
  • Coordinate and advocate for care that ensures basic needs are met. This may require coordination and advocacy with multiple local and State agencies.
  • Work with clients and their families or other support systems to ensure basic needs are met.
  • Be knowledgeable about the crisis system and know when to activate it.
  • Able to balance independent work while appropriately consulting with supervisor and taking directions when given.
  • Other responsibilities and as needed and assigned.

Strong preference will be given to candidates who graduated from a college or university with a bachelors degree in social work (BSW) AND active licensure as a Social Service Worker (SSW) in good standing in the State of Utah.

Employment Classification & Schedule

This position is classified as full-time non-exempt.

The proposed schedule for this position is Monday through Friday from 9:00 am to 5:00 pm.

Location

The DBH Main Street Clinic is located at 934 South Main Street Layton UT 84041.

While this position will have office space within the agency case management activities are expected to be performed across multiple settings including client homes and community locations. Significant fieldwork is required including frequent travel throughout the community to conduct outreach attend meetings and provide direct client services. The position may require the use of a personal vehicle; mileage reimbursement is provided in accordance with organizational policies.

Compensation

The hourly rate for this position is outlined below:

BSW Degree SSW License

  • $26.00 per hour base wage 1% per year of licensure

Degree without SSW License

  • $22.00 per hour base wage 1% per year of direct social work experience up to 10 years.

Introductory Period

All new employees at DBH have a 6-month introductory period to assess whether they can perform the essential functions of the job at the expected level within the specified timeframes and are a good fit for the job.

Required Qualifications

Education

  • Bachelors degree in social work or a related field is required.

Licensure/Certification

  • Strong preference for active licensure as a Social Service Worker (SSW) in good standing in the State of Utah. BHCM certification is not required for licensed individuals OR
  • The ability to obtain Behavioral Health Case Management (BHCM) certification within 90 days of employment is required if unlicensed.
  • The Adult Mental Health Outpatient Case Manager must complete and maintain certification in First Aid and CPR.

  • Must have reliable transportation and a valid drivers license.
    • A clean driving record is also required to utilize the DBH vehicle fleet.
  • Ability to assist clients in developing a range of social supports in the community.
  • Ability to advocate for client rights.
  • Ability to assist clients in monitoring their medications.
  • Skill in the application of problem-solving and multitasking.
  • Bi-lingual English/Spanish skills are highly beneficial.

Experience Required

While no previous experience is required previous experience working in the social work field and/or working with people who have serious mental illness is strongly preferred.

Demonstrated experience in the following areas is essential:

  • Ability to work diligently and maintain company standards without prompting.

  • Ability to follow written and oral instructions.

  • Ability to develop and maintain effective working relationships with colleagues and clients.

  • Ability to communicate effectively orally and in writing.

  • Ability to motivate and engage clients.

  • General understanding of mental illness and substance use disorders.

  • Ability to maintain client confidentiality.

The selected candidate must successfully pass a pre-employment criminal background check and drug screening. DBH will not hire individuals currently using illegal drugs or abusing alcohol; therefore making employment at DBH conditional upon successfully passing pre-employment drug screening by providing a valid sample. Candidates who are offered employment are required to report for a drug screen at an Intermountain WorkMed location within 24 hours of being requested to do so by the DBH Human Resources Department.

Davis Behavioral Health works with Federal grants and must comply with the Federal drug laws including no use of medical marijuana.

Those eligible must no longer be in active treatment at DBH and have one year of stable recovery. Active treatment includes weekly or group therapy regular case management housing peer support services etc. Clients who are in medication management services only or those who have infrequent therapy will be reviewed for eligibility on a case by case basis.

DBH utilizes an electronic medical health records system so computer literacy and familiarity with Microsoft Office products is required.

Meeting monthly face-to-face service expectations and productivity standards is also required.

The successful candidate must also have a clean driving record to operate agency vehicles and/or transport clients when necessary.
