General Summary: The Substance Use Disorder (SUD) Navigator functions as key support and coordination resource for individuals affected by substance use disorders helping them accesstimelyappropriate and continuous this capacity the incumbentis responsible forproviding a variety of substance use treatment outpatient services to members with mental illness and addiction disorders. The SUD Navigator functions include screening andidentifyingindividuals with substance use needs re-engaging members providing brief interventions and educationfacilitatingreferrals to treatment recovery and harm-reduction RESPONSIBILITIES:Maintain confidentiality of records relating tomemberstreatment. Assess members for substance use history readiness to change and level of care recommendations. Collect information about members through interviews observationor individually or in group sessions toassistin overcoming dependencies adjusting tolifeor making changes. Guide members in the development of skills or strategies for dealing with their and implement individualized recovery-oriented treatment and crisis plans based on clinical experience and with other staff members to coordinate member care and develop treatment resources or to specialists as community outreach to members as well as visit members in detox centers hospitals and other facilities to complete recovery and crisis asmembersadvocate to coordinate required services or to resolve emergency problems in crisis with families probation officers police or other interested parties to exchange necessary information during the treatment interdisciplinary team meetings to collaborate withmemberstreatment and submitdetailed datarequiredfor management of other duties as assigned or necessary as they relate to the general nature of the :Minimum Education Required:High School Diploma or Experience Required:Behavioral health experience and/or substance abuse training :Minimum 21years of age.
