Job Summary
GENERAL SUMMARY:The Community Counselor provides strength-based interventions to build self- regulation skills and develops positive coping strategies for anger impulse control depression anxiety and stress conflict resolution and general self management. Services will be provided to member and/or family in a community educational or home-based setting. Care coordination and Child and Family Team (CFT) facilitation skills will also be required and provided as needed based on the needs of the member. Community counselor may also assist in the facilitation of group counseling sessions on a rotating RESPONSIBILITIES:Provides direct family centered clinical services to members as well as care coordination to meet the needs of the member and employee productivity standards. Services include individual family and group skills building interventions and counseling services to address a variety of complex issues such as behavioral challenges parenting skills family systems issues anger management and substance abuse written diagnostic assessments assessing service recipients from multiple perspectives including family functioning psychological/emotional social academic medical behavioral substance abuse skills strengths barriers to self-sufficiency treatment history and others and utilizing various assessment Child and Family Team (CFT) meetings. Collaborates and coordinates with other service providers system partners and professionals such as attorneys school personnel social workers and probation followtreatmentplan from clinicaland billingpurposesin accordancewithagencyand relate :EducationAssociatesdegreepreferredhigh schooldiplomaorGED reliable transportation; mileage reimbursement :Minimum 18 years of Level One Fingerprint Clearance Card (must possess upon hire and maintain throughout employment).CPR First Aid & AED certification if required (must possess upon hire and maintain throughout employment).Valid Drivers License 39-month Motor Vehicle Report proof of vehicle registration and liability insurance that meet company insurance requirements if TB test result if required (Employer provides).Questions about this positionContact us
