Community Home Health Care is growingand were looking for passionate case managers to join our team. We also have an immediate opening for a Bilingual Case Manager (English/Burmese).

What Youll Do
- Client Assessments & Care Plans Evaluate needs and design personalized care plans
- Coordinate Services Partner with nurses caregivers and providers for seamless support
- Advocate for Clients Be a trusted voice for patients and families
- Maintain Compliance Keep records accurate and services aligned with state and federal guidelines
- Crisis Support Step in quickly when urgent issues arise
- Monitor & Adjust Track progress and adapt care plans to improve outcomes
What Were Looking For
- Excellent communication & relationship-building skills
- Strong organization and time-management abilities
- Knowledge of Medicaid Medicare and Indiana home care guidelines
- Background in case management social work or healthcare
- Tech-savvy with EHR or case management systems
- Bilingual (English/Burmese) is a plusimmediate need!
