JOB SUMMARY
Abernathy Home Care is hiring a compassionate and reliable RN or LPN to provide supervisory visits for HHAs in client homes. This is not an in-office position nurses travel throughout our service area. Mileage and travel time are paid. This role is perfect for nurses who value flexibility independence and meaningful client care.

ESSENTIAL JOB FUNCTIONS

• Complete supervisory visits for Home Health Aides (HHAs)

• Conduct client assessments and update care plans as needed

• Provide occasional hands-on nursing care (med setup vitals simple treatments)

• Document visits in AxisCare EMR

• Communicate changes in client condition to the care team

• Maintain professional relationships with clients families and staff

• Participate in weekend visits at least every other weekend

SCHEDULE & BENEFITS

MINIMUM QUALIFICATIONS

• Active Massachusetts RN or LPN license

• Valid drivers license and reliable transportation

• Strong assessment and communication skills

• Comfortable working independently in client homes

• CPR Certified
