Position Summary

The Myrna Daniels Seniors Emergency Medicine Centre (SEM Centre) Nursing Practitioner demonstrates autonomy while assuming and having sole accountability and the authority to independently communicate a diagnosis prescribe drugs and order a range of diagnostic tests the principal responsibilities include: utilizing and demonstrating advanced knowledge and decision making skills to conduct comprehensive assess diagnose and manage the care of patients; planning and implementing strategies to promote health and prevent illness with individuals families and groups; delivering coordinating and participating in educational and professional development activities/programs.  The SEMC NP will assume a Most Responsible Provider role in UHNs SEMC at Toronto Western Hospital which is a purpose-built clinical space to provide comprehensive and expedited geriatric consultative care and lead complex disposition planning. As such the SEMC NP will be uniquely positioned in guiding the development and evaluation of an innovative model of care for seniors requiring Emergency Medicine Care.

Duties

  • Utilizing and demonstrating advanced knowledge and decision making skills to conduct comprehensive assessments.
  • Diagnose and manage the care of patients.
  • Planning and implementing strategies to promote health
  • Prevent illness with individuals families and groups.
  • Delivering coordinating and participating in educational and professional development activities/programs.

Qualifications :

  • Completion of Masters Degree Program in Nursing (or one semester from completion).
  • Registered Nurse (EC) certification complete or pending.
  • Nurse Practitioner (NP) Adult or Primary Care certificate complete or pending.
  • Current Certificate of Competence with College of Nurses of Ontario.
  • Membership in the Nurses Association of Ontario (RNAO) organizations relevant to the role.
  • Canadian Nurses Association certification in Emergency Nursing preferred.
  • Canadian Nurses Association certification in Gerontological Nursing preferred
  • Five (5) years experience in Emergency Services preferred.
  • Clinical experience involving geriatric consultation and/or comprehensive geriatric assessments preferred.
  • Clinical experience working with community partners (OH atHome LTCHs NLOT) preferred.
  • Clinical proficiency in EPIC Electronic Health Record (EHR) systems preferred
  • Experience in nursing research methodology.
  • Ability to perform duties in a professional and courteous manner and produce high quality work while meeting deadlines in accordance to UHN standards.
  • Exercise initiative and good judgement with ability to multi-task.
  • Compliance with confidentiality requirements.
  • Effective organizational interpersonal and communication skills.
  • Knowledge of applicable legislative UHN and/or departmental policies.
  • Client service oriented with the ability to effectively work with diversity and appreciate that people with different opinions backgrounds and characteristics bring richness to the challenge or situation at hand.
  • Evidence of strong assessment skills and the ability to predict changes in patient status.   
  • Demonstrated knowledge and ability to utilize process of planning implementing and evaluating patient care.
  • Demonstrated leadership/in charge skills and evidence of good organizational and decision-making skills.
  • Evidence of commitment to team building and working within a patient focused care model.
  • X years experience working with and/or leading a multidisciplinary team preferred.
  • Program planning and evaluation methodology skills.
  • Commitment to collaborative practice.
  • Ability to utilize information technology.
  • Experience in a triage role in an emergency department setting preferred.
