Rate of Pay: 22.86- 31.55 per hour

Summary:

Westminster City Council is seeking a proactive and resilient Social Worker to join the St Marys Hospital Social Work Team. Operating within a fast-paced A&E environment this role is essential for supporting timely and safe hospital discharges for residents of the Tri-Borough. You will work to protect the rights of individuals with complex needs promoting their independence and well-being through holistic person-centered assessments and robust support planning. As part of the Westminster Way you will be expected to champion a culture of openness transparency and integrity while putting residents at the heart of every decision.

Responsibilities:

  • Conduct holistic quick assessments for Tri-Borough residents and triage referrals on behalf of the Hospital Discharge Team.
  • Undertake Care Act Assessments and develop person-centered support plans with measurable outcomes that maximize independence.
  • Lead Safeguarding Adults Investigations (Section 42.1) and act on concerns of abuse in accordance with council policy.
  • Apply the five principles of the Mental Capacity Act conducting capacity assessments and best interest meetings as a designated decision-maker.
  • Provide expert guidance and signposting to A&E staff service users and carers to enable informed choices regarding care.
  • Manage delegated budgets and set up personal budgets/direct payments ensuring all financial transactions are authorized.

Qualification:

  • A recognized Social Work qualification (CQSW DipSW or Degree in Social Work) Occupational Therapy qualification or relevant Community Nursing qualification.
  • Must hold current registration with Social Work England.

Essentials:

  • At least 2 years of experience in a Local Authority Health or voluntary sector setting.
  • Proven experience in hospital discharge work and a strong working knowledge of the Care Act and safeguarding procedures.
  • Experience working with older people or those with physical disabilities including knowledge of relevant service provisions.

  • You should be available to work immediately or at a short notice.
  • You should have right to work in U.K
  • This role requires an Enhanced DBS
