Position Purpose of a Support Worker at Focused Care:

The role is essential in supporting individuals with a range of disabilities including physical intellectual psychosocial communication and learning disabilities to live with maximum independence and dignity.

It focuses on delivering compassionate person-centred care that promotes wellbeing encourages self-determination and supports each client in achieving their personal aspirations.

The Support Worker contributes to enriching clients lives by facilitating participation in meaningful daily and community-based activities nurturing inclusion confidence and overall quality of life.

Working collaboratively with clients to establish and review daily or weekly routines that support progress toward personal goals and greater independence.

Maintaining accurate up-to-date records and progress notes in accordance with organisational and funding body requirements.

Supporting clients in developing self-advocacy and decision-making abilities and when required represent their interests appropriately to ensure their voice is heard.

Essential Criteria when applying for the position:

Right to Work in Australia A valid VISA is required to work in Australia and this must be current.

NDIS Screen Check this document must be cited prior to starting with Focused Care.

NDIS Orientation Module this module must be completed within 7 days from the start date of employment.

Suitable vehicle a vehicle that is roadworthy and has comprehensive car insurance should be in place prior to start date. The vehicle must have the capacity to transport multiple passengers (minimum 1 passenger) in a safe and comfortable manner.

Drivers License a valid Australian Drivers License must be supplied prior to start date.

COVID and Flu Vaccinations Certificate a copy of this document must be supplied prior to start date.

Current First Aid and CPR Certificate a copy of this document must be supplied prior to start date.

Working with Children Check (WWCC) a valid WWCC must be supplied prior to start date or alternatively it must be cited within 1 week from the start date of employment.

Current Manual Handling and Medication Certificates this document must be supplied before start date.

100 Points of ID evidence must be supplied prior to start date.

National Police Clearance Certificate supplied and current (maximum of 6 months old) prior to start date.
