The Inn at Menorah Park is looking for a Full Time LPN with experience in Assisted Living.
MAJOR RESPONSIBILITIES :
The following statement reflects the duties considered necessary to describe the essential functions of the job as identified and shall not be considered a s a detailed description of all the work requirements that may be inherent in the position.
- Plans safe economical efficient and therapeutic care.
- Plans and assists in reinforcing patient and family teaching and in the supervision of procedures as taught.
- Provides direct patient care especially if HHA staff is not present.
- Documents all information related to patient care.
- Recognizes changes in patient status and reports to appropriate persons.
- Acts as a liaison between patient family and physician.
- Maintains communication and coordination among all services provided.
- Assists in planning for discharge of patients when appropriate.
- Maintains timely physician orders. Verbal orders are reduced to writing and are co-signed by the physician within 90 days.
- Maintains written and verbal communication with physician regarding patient status.
- Utilizes paraprofessional personnel to assist the patient as appropriate and ordered.
- Performs clinical aide supervision according to policy and completes appropriate supervision form.
- Provides the HHA with demonstration of assigned duties and has aide re-demonstrate procedure(s).
- Communicates effectively regarding matters impacting the agency.
- Functions within the policies and procedures of the agency adheres to the mission and philosophy of the agency.
- Collaborates with supervisors to improve performance.
- Recognizes the need to provide information to other personnel as required for efficient functioning of the agency.
- Participates in activities beneficial to the growth and development of the agency.
- Functions in a professional and efficient manner.
- Maintains professional skills and continues to grow professionally.
- Operates within the scope of practice for a Licensed Practical Nurse.
- Has read and is knowledgeable of NYS Department of Health Standards for a licensed home
care agency. - Is knowledgeable on the HHA matrix developed by the NYS Department of Health and is
able to instruct IIFIA on special task per patient and document on proper form.
- Supports patient admission to Menorah Park Home Care. Works to help patients adapt to their new home.
- Discusses need for patients higher level of care with Patient Services Director not HHA.
- Works with HHA to be competent in special circumstances tasks which promote patient staying in their home.
- Schedules MD appointments and transportation at the direction of the Patient Services Director.
EDUCATION
- Current New York State license and/or registration.
- Graduate from an accredited school for Licensed Practical Nursing.
EXPERIENCE REQUIRED (MINIMUM):
- One year of experience in a hospital nursing home or community setting.
