Physical Therapist needed for North Country Home Health & Hospice Agency. Location: Groverton NH to Pittsburg NH (northern most territory). Candidates must have own vehicle and be comfortable driving in the winter weather.
NO LOCALS - Must be 50 miles from facility. Permanent address must be included in the submission file REQUIRED

Requirement description :
- 2 years current experience in Physical Therapy in Home Health REQUIRED
- age group experience: Peds to Geri
- Previous travel experience HIGHLY PREFERRED
- Home Health and/or Hospice experience REQUIRED
- OASIS experience REQUIRED
- Work History
- Physical Therapy skills checklist current within past year REQUIRED
- 1 Supervisory Signed Reference from the past year of employment REQUIRED
- Active American Heart Association BLS - REQUIREDDOCUMENT
- License verification REQUIREDDOCUMENT
- All licenses/certs must be current at time of submission
