IDD Community Team Lead

Full Time

Responsibilities may include:

Managing a team of TBS Specialist by providing professional support training and mentorship.
Managing a caseload of clients at 30% productivity.
Connecting clients with community resources.
Ensuring that the treatment/behavior plan is being followed.
Ensuring all required documentation is completed and sent to the appropriate supervisor.
Communicating and following up as needed regarding the clients status.
Working collaboratively with those providing therapy counseling and/or psychiatric services.

Requirements for this position include:

Bachelors degree in Social Work or a related field preferred
Experience in community-based case management.
Managerial experience is preferred.
Valid drivers license car insurance and reliable transportation.
Willingness to travel throughout assigned service area. This position will require regular travel to Lima Rockford Paulding and Hicksville.
