Registered Nurse (RN) Home Care Admissions & 90-Day Assessments
Position Type: Part-Time / PRN / Part Time (Flexible)
Location: Travel to clients homes within the service area (Asheboro High Point Pleasant Garden. Thomasville Randleman Trinity Troy Biscoe Star Seagrove Sophia Archdale and Ramseur)
Job Summary
We are seeking a compassionate and detail-oriented Registered Nurse (RN) to travel to clients homes to complete start-of-care admissions and required 90-day reassessments. This role is ideal for an RN who enjoys autonomy flexible scheduling and working directly with clients in a home care setting.
Responsibilities
Conduct in-home client admissions/start-of-care assessments
Perform 90-day reassessments and additional assessments as required
Complete accurate and timely clinical documentation
Assess client health status functional abilities and care needs
Collaborate with office staff caregivers and families to support care plans
Identify changes in condition and communicate concerns appropriately
Ensure compliance with company policies state regulations and best practices
Qualifications
Active Registered Nurse (RN) license in good standing
Minimum of 1 year nursing experience (home care or community-based experience preferred)
Strong assessment documentation and communication skills
Ability to work independently and manage time effectively
Reliable transportation and willingness to travel to client homes
Comfortable using electronic documentation systems
