How you will contribute
A Patient Safety Officer who excels in this role:
Leads the development implementation and ongoing evaluation of the hospitals patient safety program.
Facilitates Root Cause Analyses (RCA) Learning from Defects (LFDs) and Failure Mode & Effects Analyses (FMEA).
Oversees safety event reporting review triage and follow-up ensuring timely investigation and documentation.
Identifies trends and system vulnerabilities and partners with leaders to develop corrective actions and monitor outcomes.
Ensures compliance with The Joint Commission National Patient Safety Goals Sentinel Event Alerts and all relevant regulatory requirements.
Supports Lifepoints HRO (High Reliability Organization) principles and promotes a strong culture of psychological safety.
Provides regular patient safety reports and dashboards to the Patient Safety and Clinical Quality Committee (PSCQC) Medical Staff and hospital leadership.
Conducts proactive observational assessments safety rounds and process audits to identify operational risks.
Educates staff on patient safety principles error prevention techniques and improvement methodologies.
Collaborates with departments such as Nursing Risk Management Quality Infection Prevention Pharmacy and Medical Staff to align safety initiatives.
Maintains confidentiality of all investigations and patient information.
Performs other duties as assigned to support the hospitals safety goals.

What we are looking for
Applicants should have a bachelors degree. Masters Degree (Preferred) *Years of relevant experience may be substituted for required education.
Additional requirements include:
CPPS (Certified Professional in Patient Safety) within 2 years of hire/transfer
5 years of healthcare experience.
