Patient NavigatorJob Description
This role is crucial for delivering exceptional patient service across various clinical and community settings. It involves conducting critical health-related screenings directly with patients documenting results and communicating with patients about results. Priority screening areas include Cancer HIV and Hepatitis and health related social needs. These screenings will primarily take place in the Emergency Department setting and may also take place in other clinical and/or community settings. The position also includes navigating connection to needed services based on screening results and educating patients on prevention services and other resources. Regular communication with IMPACT staff and other relevant stakeholders as well as collaboration with the care team are essential to maintain program integrity organized records and provide culturally humble interactions.
