St. Mary Medical Center part of Trinity Health Mid-Atlantic is seeking an experienced Care Management Liaison to join our Care Management team!
This position is responsible for assisting the Care Manager and Social Worker functions with coordination of discharge planning and compliance.
Responsibilities include but not limited to:
Administration of Important Message from Medicare per Center for Medicare and Medicaid Services requirements as directed by Care Manager of Social Worker. Timely documentation to be entered the same day the form is completed.
Coordination of durable medical equipment (DME) home care home infusion and prior-authorization medications as directed by the Care Manager.
Provides patients and/or families with choice for referred services.
Coordination of dialysis faxing transportation and skilled nursing referrals at the direction of the Social Worker.
Coordination of insurance authorization for Skilled Nursing Facility and Acute Rehab at the direction of the Social Worker.
Timely and accurate completion of the PMNC for ambulance transportation when arranged.
Coverage of office duties (answering phones faxing of discharge dates management of front desk clinical request retrieval from voicemail) for Office Coordinator PTO.
Accurate documentation of discharge planning arrangements in clinical documentation system.
Arrange LYFT transportation to those patients directed by the Care Manager and/or Social Worker
Minimum Qualifications:
HS Diploma or equivalent required
Previous administrative experience preferred in the healthcare setting
Must be able to work independently and on a team
Ability to perform diversified duties with time limitations with a high degree of accuracy
