How youll contribute
The Discharge Coordinator facilitates the patients continued recovery after discharge. Provides aftercare plan coordination for patients who are in need of ongoing medical/psychosocial care following collaboration with other members of the healthcare team assesses medical needs of the patient and offers suggestions regarding follow-up care options.
Responsibilities:
Provides aftercare coordination for patients who need ongoing medical/psychosocial care following discharge.
Creates and maintains up to date aftercare coordination community resources.
Displays active involvement in treatment planning process by setting up the aftercare appointments as identified by the patients provider and therapist.
Provides resource information to patients and families as appropriate for future care needs.
Actively communicates with clients families aftercare providers and referral sources. Demonstrates proactive communication with those involved with the patients treatment.
Creates a detailed Discharge Summary and Contract for each patient as assigned.
Works well with the interdisciplinary team including physicians utilization review and nursing staff members. Demonstrates active communication with team members.
Completes all hospital and client documentation thoroughly timely and in accordance with hospital standards.
Demonstrates appropriate crisis intervention and de-escalation skills. Acts as member of team when crisis occurs assisting others to ensure patient safety.
Displays a thorough knowledge of confidentiality. Completes releases appropriately. Demonstrates knowledge of HIPAA regulations and abides by those standards.
Demonstrates a working knowledge of all clinical regulations including TJC Medicaid and state and federal laws.
Qualifications and requirements
Applicants should possess the following qualifications and skills:
Education: Bachelors degree in social work counseling psychology or relevant state licensure or at least three years of clinical experience within an inpatient behavioral health setting.
Experience: Prior experience with psychiatric and chemical dependency patients.
License:CPR certification and Crisis Prevention Training (CPI) within 30 days.
