Job Summary

Job Title:Adult Social Worker - CHC Team
Location: Blackpool(Hybrid)
Pay Rate: 30.66 per hour

Job Description:

Liquid Personnel is recruiting an Adult Social Worker to join its clients Continuing Health Care (CHC) team based inBlackpool.

What will your responsibilities be

In this role you will:

Monitor and review CHC provision including joint reviews and participation in Decision Support Tool meetings.
Undertake statutory social work duties including Care Act 2014 assessments safeguarding interventions and Court of Protection work.
Work collaboratively within a multidisciplinary CHC team to support health colleagues and deliver highquality outcomes for adults in the Blackpool community.

Qualifications & Experience:

To be successful in this role you must have:

Social Work England registration
Eligible to work in the UK
Hold a full UK driving licence
Post-qualified experience
Degree level or equivalent in Social Work
