The primary role of the LPN consists of assessing individuals current health problems and needs and participating in developing and implementing an individuals care plan. This includes symptom assessment medication and illness/symptom monitoring illness/symptom education and management and being the link between other medical providers and the individual served.

Ensure the overall health and safety of the client at all times
Address immediate medical needs of clients (phone or in person)
Maintain confidentiality of client information at all times.
Complete documentation and submit to RN weekly.
Administer medications to clients as prescribed and documented on MAR.
Complete documentation (MAR client specific monitoring) daily.
Participate in clients IDT (when necessary) and report any perceived areas of concern or accomplishments.
Complete incident reports for all unusual incidents involving the client and immediately notify RN of occurrence.
Represent the agency and client in a positive manner.
Demonstrate understanding of agency policies and procedures
Serve as a liaison between SBH and other agencies community members and medical professionals.
Work with physicians and specialists to complete plan of care
Assist in obtaining informed consent for medications/treatment
Assist in ensuring physicians orders are current and correct taking off physicians orders.
Facilitate and attend (when necessary for completion of plan of care) medical appointments.
Routine monitoring of client specific medical issues (seizures bowel habits blood pressure etc.)
Maintain weekly goal of 90% of time worked.
Other client specific duties as assigned by the RN.

Compensation: $25.02 - $28.40

Qualifications

Qualifications:

1. Current WV LPN license

2. At least 5 year experience working with I/DD population WV Waiver Program Preferred

3. Current Valid Drivers License

4. Ability to pass WV Cares Criminal Background Check.
