Job Summary

Shifts Available: 7:00AM - 3:00PM 3:00PM-11:00PM 11:00PM - 7:00AM!
Our unique Cape Code style Long Term Care Houses offer LPNs excellent resident to nurse ratiosof 1:12 on days and evenings and 1:24 for nightshifts!

Position:Licensed Practical Nurse (LPN)
Supervisor:RN Charge Nurse or Shift Supervisor
Duties:
- Responsible for the overall functioning of assigned house during shift including supervisory direction to Nurse Aides of that house.
- Establishes work assignments for unit Nurse Aides and supervises same to assure thorough task completion.
- Reports to acting Supervisor data that needs coordination with other disciplines.
- Responsible for the timely and accurate administration and documentation of physician ordered medications and/or treatments to assigned residents.
- Maintains facility procedure for the ordering and receiving of physician prescribed medications.
- Monitor for changes in residents conditions with appropriate chart documentation and refers problems areas to acting Supervisor.
- Documents in electric medical records in a timely manner according to facility policy and procedures.
- Provides shift input of resident care needs to the Interdisciplinary Care Plan for continuity of resident care.
- Maintains awareness of changing resident needs both physical and emotional. Adjusts work assignments resident priorities1 etc. to assure accepted care standards are maintained.
- Maintains documentation of shift occurrences to promote thorough repotting of resident activity to oncoming shift.
- Reports changes in residents condition to Charge Nurse or Supervisor.
- Access to entire medical record of all patients/residents.
- Consults with disciplinary team in establishing information notification to families physicians and or administration.
- Proactive in preventative/protective skin care program.
- Performs duties according to the highest ethical and moral standards and in adherence with the facilitys corporate compliance plan and all applicable governing laws. Adheres to the facilitys confidentiality statement in accessing and safeguarding protected health information (PHI) of residents and employees as necessary to perform job duties.
- Performs any other duties assigned by the shift supervisor.
- Assures completion of monthly reports as requested by Nursing Administration.
Qualifications
- Graduate of an accredited School of Practical Nursing.
- Current Licensure as a Licensed Practical Nurse in New York State.
- Leadership ability and effective communication skills.
- Demonstrates competency by completing annual job specific skills of unit.
