Erie Family Health Centers a nationally recognized top workplace with 13 sites in Chicago and suburbs is looking for a valuable addition to our Care Coordination team! The IBCCP Patient Navigator is responsible for enrolling patients into IBCCP providing patient navigation for high risk or newly diagnosed patients and coordinating support groups and events for cancer survivors and those in treatment.

Compensation is based on each candidates experience skills and education within the range identified for the role. Candidates who meet the minimum requirements of the role will start at entry in the range. Any additional skills experience and education will be reflected in the compensation offered.
Main Duties & Responsibilities
- Enroll patients in IBCCP tracks bills/budget/caseload enters into Cornerstone
- Provides case management to IBCCP enrollees diagnosed with cancer works with RN to ensure oversight.
- Provides patient navigation for high risk and newly diagnosed cancer patients primarily breast cancer but can include colorectal and cervical cancers as well
- of cases.
- Assist patients in understanding diagnosis treatment options and resources available internally and external to the organization and maintains current knowledge of available resources within the community.
- Coordinates several support groups and events including 2 monthly support groups for breast cancer two yearly mobile salons that provide items for women in treatment and 1 yearly cervical cancer and breast cancer event.
- Other duties as assigned
QualificationsEducation
- High School diploma or equivalent required
- Minimum of A.A/A.S in Community Health or Health Education LPN MA or equivalent preferred.
- Bilingual (English/Spanish) required (May be required to assist or work with non-English/Spanish only speaking patients and/or guests)
Skills and Knowledge
Required:
- At least 2 years in a healthcare setting
- Proficiency using MS Word Excel and Outlook
- Experience using electronic medical records familiarity with Cornerstone is a plus
Preferred:
- Experience with special populations especially cancer care population preferred or the strong desire to learn this population in a short period of time.
- Bilingual (English/Spanish) is a plus.
