Were looking for a Clinical Care Specialist to join our Adult Outpatient team.

What Youll Be Doing

Specifically the Clinical Care Specialist:

Makes appropriate referrals to address mental health and substance use needs
Provides behavioral health counseling group treatment and resource-based case management
Collaborates with other treatment providers to ensure a team-based approach to treatment when applicable
Identifies coping mechanisms and natural supports that may be available
Teaches and enhances client coping symptom management problem-solving independent living and socialization skills
Develops implements and reviews treatment plans with clients
Maintains competency and proficiency with our electronic health record system

Performs other job-related duties assigned by the director or his/her designee

Physical requirements include the following:

Frequently stays stationary for prolonged periods of time.
Frequently moves within and between facilities.
Frequently exposed tostressful oremergentsituations.
Frequently operatesa computer and other office equipment to record health data.
Constantly communicates with clients and others.
Occasionally moves equipment and/or materials up to 20 pounds.

Qualifications

What Were Looking ForThe Must-Haves

Bachelors degree in social work psychology criminal justice or a closely related field
experience in case management/community systems navigation
experience working with persons diagnosed with severe & persistent mental illness and substance use disorders
exceptional flexibility time management and documentation skills
Colorado drivers license a good driving record and access to transportation during work hours.
