Were looking for a Clinical Care Specialist to join our Adult Outpatient team.

What Youll Be Doing

Specifically the Clinical Care Specialist:

  • Makes appropriate referrals to address mental health and substance use needs
  • Provides behavioral health counseling group treatment and resource-based case management
  • Collaborates with other treatment providers to ensure a team-based approach to treatment when applicable
  • Identifies coping mechanisms and natural supports that may be available
  • Teaches and enhances client coping symptom management problem-solving independent living and socialization skills
  • Develops implements and reviews treatment plans with clients
  • Maintains competency and proficiency with our electronic health record system
  • Performs other job-related duties assigned by the director or his/her designee

Physical requirements include the following:

  • Frequently stays stationary for prolonged periods of time.
  • Frequently moves within and between facilities.
  • Frequently exposed tostressful oremergentsituations.
  • Frequently operatesa computer and other office equipment to record health data.
  • Constantly communicates with clients and others.
  • Occasionally moves equipment and/or materials up to 20 pounds.

Qualifications

What Were Looking ForThe Must-Haves

  • Bachelors degree in social work psychology criminal justice or a closely related field
  • experience in case management/community systems navigation
  • experience working with persons diagnosed with severe & persistent mental illness and substance use disorders
  • exceptional flexibility time management and documentation skills
  • Colorado drivers license a good driving record and access to transportation during work hours.

  • Experience successfully working in multi-disciplinary teams
