What Youd Be Doing:

• Administering screenings assessments and facilitating collaborative treatment planning;
• Providing individual couples/family and/or group services;
• Maintaining accurate compliant and up-to-date client records;
• Participating in team meetings case consultations and trainings;
• Meeting performance standards related to documentation caseload management and productivity;
• Working collaboratively and representing Health Solutions with community agencies and partners; and
• Performing other job-related duties as requested and assigned based on program and client needs.

• May stay stationary for prolonged periods of time.
• Moves within the facility.
• Frequently exposed tostressful oremergentsituations.
• Frequently operatesa computer and other office equipment to record health data.
• Constantly communicates with clients and others.
• Occasionally moves equipment and/or materials up to 20 pounds.
• Occasionally drives between Health Solutions facilities.

What Were Looking ForThe Must-Haves

• Masters degree in social work counseling or a closely related field;
• Willing and able to obtain licensure candidacy status within 1 month of hire and clinical licensure within 3-years of hire;
• Experience working successfully within a multi-disciplinary team;
• Computer proficiency;
• Valid Colorado drivers license good driving record and access to transportation during work hours.

What Wed Like to SeeThe Nice-to-Haves

• Independent Colorado clinical license (i.e. LPC LCSW LMFT)
• Colorado Addictions Credentials (i.e. CAT CAS LAC)
• Spanish language skills
