Department/Unit Summary
Transitional Care Management Nurse coordinating care for patients transitioning from inpatient care or facility-based care to the outpatient setting. This role focuses on reducing hospital readmissions ensuring continuity of care and supporting patients during high-risk post-discharge periods in compliance with CMS TCM as an advocate for the physical and emotional well-being of the patient.

Qualifications and requirements
As supervised and delegated by a registered nurse provides direct patient care in accordance with applicable scope and standards of practice.
Education: Graduate of a Practical Nursing program.
Licenses: Current LPN License to practice in the State of Arkansas.
Certifications: AHA Basic Life Support (BLS) within 30 days of hire
Required Skills:
Requires critical thinking skills decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
