Job Duties
Under the direction and supervision of the Benefits Coordinator the Transportation Coordinator will assist with discharge transportation for Civil TDO patients. They will be assisting with discharge needs related to all Civil patients and assuring these transportation/discharge needs are addressed in a timely manner. This position will help ensure that the quality of patient care provided to individuals with serious mental illness and their families continues. The Transportation Coordinator will provide confidential efficient and successful transition to discharge placements to supporting service provider appointments and programming.
Minimum Qualifications
* Valid Drivers License is required.
* High school diploma or equivalent.
Additional Considerations
* Will need to be available for discharge transportation outside of normal work schedule as needs arise.
* Experience in an inpatient psychiatric healthcare facility acute care long-term care or correctional setting is preferred.
* Prefer experience in case management discharge planning and collaboratively working with various community social service and behavioral health agencies.
* Experience and knowledge of benefits/application processes preferred.
* Associates degree or higher in human services social work or related field preferred
