Home Health Speech-Language Pathologist (SLP)

Part-Time 2 Days per Week
Service Area: Bay City Midland Saginaw

At Elara Caring youll work one-on-one with patients in their homes no crowded clinics no overlapping sessions just focused intentional care in a setting where patients are most comfortable.

What Youll Do

• Complete in-home evaluations for speech language cognition voice and swallowing

• Develop and implement individualized treatment plans aligned with physician orders

• Educate patients and caregivers on communication strategies and home exercise programs

• Collaborate with nurses therapists and physicians to ensure coordinated care

• Document services accurately and in a timely manner

• Support patient safety and help reduce unnecessary hospitalizations

What Were Looking For

• Masters degree in Speech-Language Pathology from an accredited program

• Active state SLP license (or Educational Limited License with supervision)

• Prior SLP experience preferred; home health experience a plus

• Strong organizational skills and ability to work independently

• Compassionate patient-centered approach to care

• Reliable transportation and valid drivers license
