About the Role
An opportunity currently exists for a Grade 1 Social Worker to join our team in acute at St Vincents Hospital Fitzroy. To fulfil this role it is essential that you have current experience working in an inpatient hospital social work position post completion of your social work qualification. This could be in a sub-acute setting however experience in an acute setting will be highly regarded.
To perform this role you will:
Use your skills to undertake multi-dimensional comprehensive psychosocial assessments apply clinical reasoning and formulate appropriate complex intervention and discharge plans
Liaise with patients their families and/or carers to co-ordinate facilitate and communicate patient treatment and outcomes
Actively participate in quality improvement and research activities
Promote and develop positive relationships within the department wider organisation and with community partners
Classifications for Grade 1 Social Workers range from SC13 SC16 ($74172 - $88872 per annum).
Your Contribution
Bachelor Degree or Qualifying Masters in Social Work
Social work experience in a hospital inpatient care setting post graduation is essential
Current National criminal history check or willing to obtain
Current Working with Childrens Check desirable
