Scheduled hours: Day - Varies Occasional Weekend RequirementsJob Description:
Provides basic patient care under the direction of a licensed nurse. Serves as patients Personal Care Champion by assisting them with activities of daily living. Provides care & support in the home setting or hospice house with special attention to patient personal care needs in end of life care. Perform visits and/or shift work. Works independently providing personal cares to individuals in their homes and facilities. Must be able to work independently have good time management skills timely documentation and a valid drivers license.
Qualifications:
Prefer Iowa Certified Nurses Aide
Maintains valid licensure/certification/registration if required
Current Drivers license and proof of insurance for staff performing visits.
