The LPN provides direct student healthcare administering meds first aid and managing chronic conditions in collaboration with the Licensed School Nurse. supporting the student in their learning and RequirementsValid Vermont LPN LicenseCurrent CPR CertificationGraduation from an accredited LPN programHealthcare Duties to Include:Be knowledgeable of health conditions and related health care with the school RN to assist the family to identify the students health related barriers to with the School Nurse and medical care team to provide input on care plan development Communicate with the School RN students family and health care providers to exchange relevant information. Communicate with appropriate school personnel relevant information as necessaryProvide specialized medical interventions according to the Individual Health Care Plan (IHP) healthcare provider orders and CCSU protocols Provide personal care such as: incontinence care repositioning hygiene maintenanceProvide emergency care as per the student emergency action planAccurately take vital signs when indicatedDocument care appropriatelyOther duties as assignedAcademic DutiesSupport students academic needs in the educational environmentWork with SLP to learn to utilize communication devicesAssist student in utilizing adaptive communication toolsMay require some development of low-tech materials such as: flashcards
