Provides comprehensive behavioral health services including assessment diagnosis treatment planning and psychotherapy using evidence-based practices. Delivers culturally responsive care to individuals families and groups while ensuring patient safety regulatory compliance and effective collaboration with multidisciplinary teams.

• Conduct psychosocial and diagnostic assessments including collateral information as clinically indicated

• Diagnose patients in accordance with the DSM

• Develop implement and monitor individualized treatment plans

• Provide individual family couples group and crisis psychotherapy

• Identify and respond to risk of harm to self or others

• Refer patients to additional services when appropriate

• Maintain timely accurate clinical documentation and meet productivity standards

• Collaborate with families external providers and care teams

• Participate in required trainings meetings and supervision activities

• Adhere to ethical standards HIPAA and all federal state and organizational regulations

• May complete court commitment reports EAP services or provide clinical supervision as assigned

Minimum Qualifications

• Masters degree in Social Work Counseling or Marriage and Family Therapy from an accredited program

• Active clinical licensure:

  • Iowa: LISW LMFT LMHC

  • Illinois: LCPC LISW LCSW LMFT

  • Wisconsin: LCSW LMFT LPC

• Minimum of 2 years post-graduate clinical experience

• Eligibility to bill government payers

• Ability to maintain licensure and required certifications (BLS may be required by site)
