HOSPICE OF THE MIDWEST is seeking a compassionate and creative Music Therapist in Forest Lake MN. As a Music Therapist You will:
- Assess patients functional status musical responses and preferences
- Develop individualized treatment plans to help patients meet goals
- Deliver music therapy using instruments singing movement and listening
- Monitor and evaluate patient progress regularly and update care team
- Maintain accurate clinical records and communicate changes in patient condition
- Collaborate with interdisciplinary team members to ensure coordinated care
- Participate in meetings training and quality improvement initiatives
- Educate patients families caregivers and staff on treatment plans

Qualifications
- Bachelors degree or higher in music therapy from an AMTA-approved program
- Board-certified (MT-BC) by CBMT
- Preferred: 2 years clinical music therapy experience
- Valid drivers license with insured reliable vehicle
- Strong verbal written communication and organizational skills
- Understanding of hospice philosophy and end-of-life issues
