Responsibilities:

Oversee and coordinate tailored care plans ensuring that our clients receive appropriate and timely services.
Serve as a strong advocate for clients ensuring their needs are met in home medical and care facility settings including while communicating with healthcare providers.
Work closely with team members to address clients needs and contribute your expertise to the team helping to enhance the collective knowledge and skills.
Participate in after-hours or weekend on-call rotations as needed to provide continuous support to clients.
Update family members and other members of the care team and maintain thorough and accurate documentation. Approximately 90% of the role involves field work with 10% dedicated to follow-up and office-based tasks.

Requirements:

A masters degree in a related field or a bachelors degree with at least 2 years of relevant experience in gerontology mental health home care or similar.
Valid Drivers License
Valid Virginia RN License
Available 9am to 6pm Monday-Friday and some weekends.
Dependable adaptable and accountable with a strong commitment to providing exceptional care and support to clients.
Must be able to lift and maneuver up to 50 pounds.
Must be comfortable and willing to drive regularly around the Virginia and DC Metro area.
